Below, you will notice that there are two different order forms. This is because our products fall into one of two categories:

Normal Order Form (Makawao Elementary School T-Shirt Order Form) - This form includes Cotton Uniform T-Shirts, Dri-FIT Uniform T-Shirts, Cotton Uniform Long Sleeves, and Dri-FIT Uniform Long Sleeves in sizes Youth Small to Adult Extra Large.

Specialty Order Form (Makawao Elementary School Specialty Order Form) - This form includes Hoodies, Zipper Jackets, and sizes 2XL+ for Cotton Uniform T-Shirts, Dri-FIT Uniform T-Shirts, Cotton Uniform Long Sleeves, and Dri-FIT Uniform Long Sleeves.

Please only fill out the necessary forms for the products you'd like to order. For example, if you want to order 5 Cotton Uniform T-Shirts and a Hoodie, you would need to order the 5 Cotton Uniform T-Shirts through the Normal Order Form, and the Hoodie through the Specialty Order Form.
